Maine, Nevada, Rhode Island, Virginia – Beginning January 1, 2010, Maine, Nevada, Rhode Island and Virginia will only accept IACA approved forms UCC1ad and UCC5 versions dated 5/21/09. Previous versions of the forms will be rejected. Note that the title of the UCC5 form has been changed from “Correction Statement” to “Statement of Claim.”
